To set up a wholesale account to resell our products, we will require a copy of your business license or other documentation showing your federal tax ID number, as well as your reseller permit (if applicable). Massachusetts companies, please include a signed ST-4 Sales Tax Resale Certificate (MA-dor-sales-use-form-st-4).
